A client may have normal fasting glucose, struggle with persistent hunger and abdominal weight gain, and still be told that their blood sugar is “fine.” This is where the distinction between insulin resistance versus diabetes becomes clinically and commercially relevant for wellness professionals. One describes a metabolic pattern that can develop over time; the other is a medical diagnosis requiring appropriate clinical management.
For practitioners, understanding the difference creates a better client experience. It helps you avoid oversimplified advice, recognize when a referral is needed, and build nutrition support around the individual rather than a generic low-carb or calorie-restriction plan.
Insulin Resistance Versus Diabetes: The Core Difference
Insulin is a hormone that helps move glucose from the bloodstream into cells, where it can be used or stored. With insulin resistance, muscle, liver, and fat cells become less responsive to insulin’s signal. The pancreas may compensate by producing more insulin to keep blood glucose within a normal range.
That compensation is the key point. A person can have insulin resistance for years without meeting diagnostic criteria for diabetes. Their glucose values may appear relatively stable while elevated insulin requirements, appetite changes, energy fluctuations, triglycerides, waist circumference, or liver markers suggest growing metabolic strain.
Diabetes, by contrast, is diagnosed when blood glucose reaches defined thresholds or when other accepted diagnostic criteria are met. A broader assessment may include fasting glucose, HbA1c, triglycerides, HDL cholesterol, liver enzymes, blood pressure, waist circumference, health history, medication use, sleep quality, activity patterns, and family history. In some cases, regulated health-care providers may also assess fasting insulin or calculate insulin-resistance markers. Interpretation should always sit within professional scope and alongside the client’s physician or nurse practitioner when indicated.
In Canada, diabetes is commonly diagnosed using measures such as an HbA1c of 6.5% or higher, fasting plasma glucose of 7.0 mmol/L or higher, a two-hour oral glucose tolerance test result of 11.1 mmol/L or higher, or a random plasma glucose of 11.1 mmol/L or higher in the presence of classic symptoms. Testing and confirmation requirements can vary by clinical situation.
A wellness practitioner should not diagnose diabetes or promise to reverse it. Your opportunity is to recognize risk, support sustainable food and lifestyle habits, and refer clients for medical assessment when blood work, symptoms, or history warrant it.
The Client Presentation Is Often More Nuanced Than a Label
Clients rarely arrive saying, “I believe I have insulin resistance.” More often, they describe a familiar cluster of concerns: fatigue after meals, intense cravings, difficulty managing weight despite repeated dieting, disrupted sleep, irregular meal patterns, or a family history of type 2 diabetes.
These signs are not diagnostic on their own. Fatigue and weight changes can have many causes, including inadequate sleep, stress, thyroid conditions, medication effects, under-fuelling, menopause transitions, and other health concerns. That is why practitioners need a structured intake process rather than relying on a symptom checklist or a single lab value.
The same principle applies to body size. Insulin resistance can occur across body types, while not every larger-bodied client has the same metabolic risk profile. A respectful, evidence-informed approach looks at the whole picture: lab values, medical history, eating patterns, personal goals, and capacity for change. It does not reduce a client’s health to a number on the scale.
Type 1 Diabetes and Gestational Diabetes Are Different Conversations
The phrase “diabetes” is often used as if it refers only to insulin resistance. It does not. Type 1 diabetes is an autoimmune condition in which the body produces little or no insulin. It requires medical management and insulin therapy. Gestational diabetes develops during pregnancy and also needs timely medical oversight because it affects both parent and baby.
For these clients, nutrition coaching can be valuable as part of the care team, but it must never replace individualized medical guidance. Any client using insulin or glucose-lowering medication needs particular caution around significant dietary changes, fasting, or increased activity because medication requirements and hypoglycemia risk may change.
Nutrition Support Should Address the Pattern, Not Just Restrict Food
The temptation is to treat insulin resistance as a simple carbohydrate problem. Carbohydrate quality, quantity, timing, and individual tolerance can matter, but overly restrictive plans often create poor adherence, rebound eating, and a strained relationship with food.
A more effective framework considers meal composition and consistency. Protein, fibre-rich carbohydrates, vegetables, healthy fats, and appropriate portions can support satiety and more stable post-meal glucose responses. Regular meals may help some clients reduce reactive snacking, while others benefit from a different structure based on work schedules, training demands, medication use, and personal preference.
The trade-off is clear: a highly prescriptive plan may produce short-term compliance, but it may not be sustainable. A vague “eat healthier” approach can feel supportive, yet fail to provide enough direction for meaningful change. The strongest practitioner model combines clear structure with personalization and ongoing adjustment.

Rather than assuming every client with elevated weight or cravings needs the same protocol, practitioners can use relevant blood values and health history to identify priorities, create realistic meal plans, and monitor progress in a more organized way.
A Practical Framework for Wellness Professionals
When a client presents with possible insulin resistance or a known diabetes diagnosis, begin by separating what you know from what needs medical clarification. Review available labs, ask about diagnoses and medications, and identify whether the client has a prescribing clinician involved in their care.
Then establish a support plan around the factors within your scope: food quality, meal structure, grocery routines, preparation skills, activity habits, sleep, stress management, and adherence barriers. For a client with diabetes, ensure your recommendations align with medical advice and encourage them to discuss significant changes with their primary-care provider or diabetes team.
Progress should not be measured only by weight. Depending on the client’s goals and available data, meaningful indicators can include meal consistency, energy, hunger levels, waist measurements, activity capacity, sleep, digestive comfort, follow-up labs ordered by their medical provider, and confidence in maintaining new habits.
